Open Dental stays your practice management system and your chart. This adds e-signed consent forms and files the signed copies of your office's own consent templates into Open Dental's own Imaging module through Open Dental's API. It is not a second chart you have to check.
Send the consent ahead of the visit or hand the patient a tablet in the chair. The patient enters their full name and date of birth, reads the consent, fills in any fields you added, and signs. The signed PDF is stored right away and the office is emailed a link to it (one summary email when the consent is part of an intake packet), so the signature is captured whether or not the chart filing goes through.
We match on name and date of birth. If the date of birth doesn't match any chart, we fall back to the first chart with that exact name, and that fallback does not check for a second patient with the same name, so a mistyped birth date on a common name can still land on the wrong chart. When a consent arrives without a date of birth and two patients share that exact name, we file nothing. No match means no upload, and a consent never creates a new chart.
The folder is chosen by name from your own image categories: Patient Information first, then Miscellaneous, whichever your office actually has. The description is the consent's title, for example 'Implant Surgery Consent - Dr. Thomas', so staff see the consent's title next to the document date. This filing step, with the by-name folder choice, has put intake PDFs into live Open Dental charts. Send a test consent and check where it lands.
Before this, a paper consent meant signing, scanning, finding the patient in Imaging, picking a folder, retyping a description and shredding or boxing the paper. I watched staff do that, and I watched them re-file PDFs that landed in the wrong folder. Now, for a matched patient on one of your templates, the file is in the chart before the patient leaves the chair, in your Patient Information folder (or Miscellaneous if you don't have one). If your office keeps consents in a different folder, staff will still need to move it.
If you already connect Open Dental to us, there is nothing extra to set up for consent forms. If you don't, you turn on Open Dental's API, paste in the key we give you, and confirm the eConnector is running on your server. That takes about five minutes, and nothing gets installed on your workstations. Then load your consents (extraction, implant, endo, sedation, financial, whatever you use) as your office's own templates. Our built-in library consents file to the chart only when signed inside an intake packet; sent on their own, they are stored and emailed to the office but not filed. So if you want a consent in Open Dental every time, set up your version as an office template. Send a test one to yourself and check which folder it landed in. Consents go to Patient Information, or to Miscellaneous if you don't have that folder. We can't point them at a different folder name per office yet.
Consents built from your office's own templates do, when the patient can be matched to an Open Dental chart. The signed PDF is uploaded into that patient's Imaging module (Open Dental's document center) with no scanning or attaching. Our built-in library consents file only when signed as part of an intake packet; sent as a standalone link, they are stored and emailed to the office but not filed to the chart yet. If there's no match, nothing is filed. We don't currently alert you when that happens, so have someone check new signed consents against the chart until you trust it. The office still has the emailed link (good for 7 days) and the copy stored in your dashboard to attach by hand.
In the patient's Imaging module. We look for a folder named Patient Information among your image categories, then Miscellaneous, and use the first one you have. The description is the consent's title, so staff see something like 'Implant Surgery Consent' next to the document date. Send a test consent to confirm the folder at your office.
By folder name, checked against your office's own image categories each time we upload. Every office's category numbers are different, so a fixed number would file into the wrong folder. If none of the names we look for exist, we upload without a category and Open Dental puts the file in your first folder. We can't set a different folder name per office yet. That's why the setup step says to send a test consent and check where it lands.
Treatment consents file when they are set up as your office's own templates. The built-in library versions (extraction, root canal, anesthesia, bone graft, crown and bridge) file only when signed inside an intake packet. Signed treatment plans go through the filing step into a Treatment Plans folder if you have one, otherwise Patient Information or Miscellaneous. Medical clearances, referral letters and insurance verification records go through it as well.
On name and date of birth, and the consent form asks for both before the patient can sign. If the date of birth doesn't match a chart, we fall back to the first chart with that exact name and don't check for duplicates, so a mistyped birth date on a common name can file to the wrong patient. If a consent arrives without a date of birth and two patients share that exact name, we file nothing. An accurate date of birth on the consent and in Open Dental matters.
No. A consent only files to a chart that already exists. New patients are created from the intake form, which checks for an existing chart first. If a new patient signs a consent before their chart exists, file that consent by hand.
No. We file a signed PDF into Imaging. We don't create an Open Dental Sheet or eForm, and we don't write the answers into structured chart fields. The signed document is the record, and it's in the chart where your team expects it.
Health-update PDFs use the same filing step as intake PDFs, which we've seen work on live Open Dental charts. Health updates are also built to add a commlog note. Our Open Dental offices haven't submitted a health update yet, so neither the health-update PDF nor its commlog note has been seen on a live chart.
You don't need Open Dental's own web forms or eClipboard. The connection does need the eConnector running on your server, which is the same service eServices uses. If you already use eServices it's probably running. If not, it's part of setup. Whether you still need two forms tools is something to decide after you've used both.
We sign a BAA on every plan. Patient data is encrypted at the field level when stored, access is limited to your practice, and you can disable the Open Dental API key from inside Open Dental whenever you want.
If your consents are still paper, or e-signed somewhere and then downloaded and attached, this removes the scan-and-attach step for consents you set up as your office's own templates. The signed PDF goes into the matched patient's Open Dental Imaging module, in your Patient Information folder (or Miscellaneous), titled with the consent's name. When the patient can't be matched, nothing gets filed. We don't currently alert you when that happens, so have someone check new signed consents against the chart until you trust it.

You keep Open Dental. Patients sign consents on a tablet or their phone. When the consent is one of your office's own templates and we can match the patient, the signed PDF goes into that patient's Open Dental Imaging module, in your Patient Information or Miscellaneous folder, titled with the name of the consent. Nobody has to print, scan, rename or drag a file into the chart. The same filing step handles signed treatment plans, medical clearances, referrals and insurance verification records. Our built-in library consents (extraction, root canal, anesthesia, bone graft, crown and bridge, general treatment, HIPAA) file to the chart when they're signed as part of an intake packet. Sent on their own as a standalone link, they are stored and emailed to the office but don't file to the chart yet. Your office's own templates file either way.
